Hospice Services

Pointe Coupee Hospice has been in operation since 2001 and offers our most experienced nursing team for pain management and symptom control. Hospice offers a team approach to manage a patient’s medical, emotional, and spiritual needs. Care is patient and family centered to meet individual needs. Our hospice team believes that every person has a right to dignified and pain-free end-of-life. We work closely with our hospice physician, Dr. Ashley Bordelon, as well as your primary care or specialty physician to manage symptoms. Medical social work, chaplain, and volunteer services are hospice care components that help to support patients and families emotionally, spiritually, and socially. Hospice care can also include many of the other home health services that you are familiar with such as physical, speech & occupational therapy.

Hospice care is most often provided where the patient lives, whether in their own home, the home of a family member, a nursing home or assisted living facility. Inpatient hospice care in the hospital can also be provided if most appropriate.

Our team provides comforting and compassionate care to patients and their families with a goal of enhancing quality of life at a time when every moment shared and memory made is most precious.

A TEAM APPROACH TO COMPASSIONATE CARE

Our hospice care is delivered through an interdisciplinary team approach that addresses each patient's physical, emotional, social, and spiritual needs. Our team is committed to ensuring that every person experiences comfort and dignity, with the highest possible quality of life. We work closely with our Hospice Medical Director, Dr. Ashley Bordelon, to expertly manage pain and symptoms while providing compassionate support throughout the hospice journey.

Hospice Medical Director
Provides medical oversight, adjusts medications, collaborates with the hospice team, and ensures each patient receives compassionate, individualized care.

Nurse Practitioner
Evaluates patients, manages symptoms, and works closely with the medical director to enhance comfort and quality of life.

Hospice Nurse
Makes routine hands-on nursing visits to assess patient and family needs, coordinate each patient's plan of care, manage pain and symptoms, monitor patient’s comfort, educate families, and offer ongoing support.

Certified Nursing Assistants (CNAs)
Assists with personal care, including bathing, dressing, grooming, and other daily activities, while promoting comfort and dignity.

Medical Social Worker
Provides emotional support, counseling, community resources, advance care planning assistance, and guidance for patients and families navigating end-of-life care.

Chaplain
Offers spiritual care, emotional support, and guidance that respects each patient's beliefs, values, and traditions.

Trained Volunteers
Provides companionship, caregiver respite, and emotional support to enhance the quality of life for patients and families.

Physical, Occupational, Speech Therapist(s)
Our therapists promote comfort, safety, and independence through gentle, individualized care. They educate caregivers on safe techniques for transferring, bathing, and repositioning patients. Every intervention is designed to improve quality of life while supporting both the patient and their family.
