Pointe Coupee Homebound Health has been serving the citizens of Pointe Coupee, as well as parts of Iberville, West Baton Rouge, West Feliciana and Avoyelles parishes for over 35 years. We have a proven record of success. Pointe Coupee Homebound Health has also been named a 2018 HHCAHPS Honors Elite recipient, a prestigious award recognizing home health agencies that continuously provide quality care as measured from the patient’s point of view.
Our staff is highly professional and provides service above and beyond what is required. Pointe Coupee Homebound Health Services provides Nursing Care, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker services, and Personal Care Attendant services for patients with a wide variety of illnesses and conditions.

Wound care including management of negative pressure devices

You May Need Home Health Care if You Are Considered Homebound and you are:
Frequently hospitalized or visiting the emergency room
Taking multiple medications with frequent medication changes
Forgetting to take medications and/or injections or need instructions on administering your own injections
Having a decline in personal hygiene including trouble dressing or bathing
Recently home from the hospital or nursing home
Experiencing a notable weight loss
Recently diagnosed with a new condition or illness
Experiencing unstable vital signs, such as an uncontrolled blood pressure
Experiencing a problem with low vision
Having a joint replaced or heart surgery
In need of wound care

Pointe Coupee Hospice has been in operation since 2001 and offers our most experienced nursing team for pain management and symptom control. Hospice offers a team approach to manage its patient’s medical, emotional, and spiritual needs. The care is patient and family centered. Hospice believes that every person has a right to dignified and pain-free end-of-life. We work closely with our hospice physician, Dr. Ashley Bordelon, as well as your primary care or specialty physician to control pain and symptoms. Hospice offers a spiritual and volunteer component that makes it unique to other health care. Hospice also offers many of the other home health services that you are familiar with such as physical, speech & occupational therapy, and a medical social worker.
We work with patients and families with end-of-life issues offering comforting and compassionate care. Hospice is not limited to only the home but may also be provided in the nursing home and hospital.

The Pointe Coupee Nutrition Program provides a complete education/training experience based on the needs of the patient and the recommendations of the physician. The Pointe Coupee Nutrition Program has a Registered and Licensed Nutritionist and Certified Diabetes Educator available by appointment.
