“FIVE WISHES” SEMINAR
Pointe Coupee Homebound Health Services and Pointe Coupee Hospice will host a “Five Wishes” seminar on Wednesday, February 29, 2012 from 10:00 am to 11:30am. Please call (225) 638-5717 to pre register. Light refreshments will be served. Participants will be given a free “Five Wishes” booklet and watch an information DVD on how to fill it out.The seminar will be facilitated by nurses, a social worker, hospice volunteer and chaplain.
“Five Wishes” is a living will comprised by “Aging with Dignity” that talks about your personal, emotional and spiritual needs as well as your medical wishes. It lets you choose the person you want to make health care decisions for you if you are not able to make them for yourself. Five Wishes lets you say exactly how you wish to be treated if you get seriously ill. It was written with the help of The American Bar Association’s Commission of Law and Aging, and the nation’s leading experts in end-of life care. It’s also easy to use. All you have to do is check a box, circle a direction or write a few sentences.

Pointe Coupee Homebound Health Services has received a 2010 Louisiana Home Health Capstone Quality Award presented by eQHealth Solutions, the Medicare Quality Improvement Organization for Louisiana. With this award, Pointe Coupee Homebound Health Services has been recognized for improving the quality of health care given to their patients, and Pointe Coupee Homebound Health Services is one of 17 home health agencies in the state to receive the 2010 Louisiana Home Health Capstone Quality Award.
Previously titled the Louisiana Home Health Quality Awards (platinum, gold, silver and bronze levels), the Louisiana Home Health Capstone Quality Awards recognizes Louisiana home health agencies that have excelled at reducing avoidable acute care hospitalizations (ACH).
The enclosed 2010 Louisiana Home Health Capstone Quality Award Fact Sheet provides detailed information regarding this award, which was announced at the 2011 Louisiana Medicare Quality Summit hosted by eQHealth Solutions in Baton Rouge on April 29.
The clinical topics measured for the awards have been designated as national health care priorities by the Centers for Medicare & Medicaid Services. Staff members from Pointe Coupee Homebound Health Services have been working with quality improvement specialists from eQHealth Solutions to use proven, evidence-based practices to improve care for their patients.

Diabetic clients may meet with a certified diabetes nurse educator and a licensed dietician for assistance in learning diabetes self management skills.
